Grants to the Community
11th Street Family Health Services Center
of Drexel University – $54,000
grant to support the primary care behavioral health model of care
for children and their families in North Philadelphia, providing
integrated behavioral health services in primary care by primary
behavioral health consultants.
The Adoption Center of Delaware Valley – $10,000 grant to
support the child preparation program, involving children in the
process of family selection and assisting public and private agencies
and social workers in the Philadelphia area to adequately prepare
children for the family recruitment process and match children
with adoptive families.
AIDS Law Project of Pennsylvania – $15,000 grant for second
year of funding to provide free legal counsel and representation
to clients in securing all healthcare benefits to which he or she
is entitled.
ALS Association, Greater Philadelphia
Chapter – $50,000
grant to support the In-Home Care Program, providing free personal
care by home health aids to patients in the advanced stages of
ALS living in the Philadelphia region.
Anti-Violence Partnership of Philadelphia – $20,000 grant
to support the Counseling Center’s child victim specialist
to provide individual and group therapeutic counseling to children,
youth and adults traumatized by exposure to violence.
Bethesda Project - $60,000 grant for a full-time registered nurse
to provide care to and oversight of the health needs of formerly
homeless people at Bethesda Project residential facilities throughout
Philadelphia.
Bridging the Gaps – $40,000
grant to support the Philadelphia Community Health Internship Program
to teach future health professionals about the reciprocal connections
among the health care system and the health of the population.
Child Abuse Prevention Effort (CAPE) – $20,000 grant to
support the Emergency Family Fund to provide essential emergency
needs for children such as food, clothing, beds and bedding, school
supplies and uniforms and shelter; and free parenting and child
abuse prevention workshops for parents and professionals.
CHOICE (Concern for Health Options: Information,
Care and Education) – $10,000
grant for the expansion of the health workshops and education and
outreach effort to youth in the Latino communities throughout Philadelphia.
Congreso – $45,000 grant to support the Un Buen Comienzo
(“A Good Beginning”) program in addressing the mental
health needs of pregnant and parenting women through enhanced in-home
clinical services provided by master’s level clinicians.
Crozer-Chester Medical Center – $40,000 grant for a second
year of funding the Youth Leadership Program, which trains high
school students in the Chester Upland School District to be peer
leaders and community health educators in order to address medical
and social issues that negatively impact the health of students.
Deaf-Hearing Communication Centre – $5,000
grant to support a comprehensive education and outreach program
for health care providers in Delaware County hospitals and health
care facilities to enhance two-way communication between them and
deaf and hard-of-hearing health care consumers.
Federation Early Learning Services – $52,000 grant to support
an early intervention program for preschool children with special
needs in the Greater Philadelphia region. A pediatric occupational
therapist assesses children in the classroom and develops strategies
to meet the children’s special needs. Team meetings also
are held with all stakeholders, including parents, teachers and
school administrators.
The Food Trust – $20,000 grant for the Healthy Times newspapers
written and illustrated by students in Norristown, Philadelphia
and Upper Darby schools to promote health, fitness, nutrition and
safety in schools and after-school programs.
Healthlink Medical Center – $36,000 grant to develop an
electronic medical record database to reduce costs and increase
procurement of medications for patients.
Institute for Safe Families – $33,000
grant to improve health care identification of and response to
domestic violence through a self-reported screening within four
Philadelphia Department of Public Health community health centers.
Keystone Hospice – $60,000 grant to care for indigent hospice
patients in their homes and in the Keystone House.
Kids Smiles – $30,000 grant for the expansion of the Kids
Smiles’ Southwest Philadelphia center and the expanded functions
of a dental assistant and an in-house educator.
Maternity Care Coalition – $35,000 grant for the MOMobile
at Riverside Correctional Facility to provide women who are pregnant
or mothers of young children with health education, parenting skills
training, individual counseling and group sessions.
Philabundance – $15,000 grant for the Children’s Hunger
Initiative to provide heat-and-serve meals, meal ingredients and
snacks to area agencies that serve low-income children.
The Sayre Health Center – $60,000 grant to support a sustainable
model of university-school-community partnerships for improved
health promotion and disease prevention that utilizes University
of Pennsylvania students at the center’s school-based health
clinic.
St. Helena Friendly Visitor Program – $25,000 grant to support
the Friendly Visitor Program for frail, isolated seniors living
in the Olney/Logan neighborhoods of Philadelphia.
Support Center for Child Advocates – $20,000 grant to provide
programs and training workshops for volunteer attorneys and paralegals
who in turn provide free legal services to help improve the health
and well-being of medically needy children in Philadelphia who
are subject to protective proceedings in Family Court.
The Clinic – $56,100 grant for a second year of funding
for a full-time staff nurse at The Clinic in Phoenixville, Chester
County. The clinic’s volunteer medical professionals provide
comprehensive primary and specialty services.
University of Pennsylvania – $40,000 grant to support the
Urban Nutrition Intern Program, which involves high school students
in year-round job training and skills development through urban
gardens, youth-run farm stands and the teaching of families to
prepare affordable, healthy meals in low-income Philadelphia neighborhoods.
Wireless Philadelphia – $50,000 grant for the TEACH (Training,
Education, Access, Content and Hardware) pilot collaboration program
with the City of Philadelphia’s Division of Maternal, Child
and Family Health to increase access to medical and health information
on the Internet. This allows individuals to be more aware of issues
affecting their own health and the health of their families.
Pennsylvania Hospital Program Grants
Department of Obstetrics and Gynecology – $60,000 grant
for a second year of funding the Latinas Health Services Program
to provide diagnostic clinical screenings and prenatal care for
Mexican women immigrants who are uninsured and ineligible to receive
Medicaid benefits. The program is a collaborative effort between
Pennsylvania Hospital, Women and Children’s Health Services
and Puente de Salud and JUNTOS, community advocacy/outreach organizations.
Diabetes Education Center – $57,233 grant for a second year
of funding the Healthy Futures Education Program, which provides
education and support for the ever growing numbers of diabetes
patients who come to the center for care.
Family Education Department – $51,000 grant for a lactation
consultant to provide services for the Women and Children’s
Health Services Breastfeeding Education and Support Program. The
lactation consultant visits those who have chosen not to breastfeed,
attends prenatal childbirth classes, visits patients after they
give birth and provides technical support and assistance with breastfeeding.
Hall Mercer Community MH/MR Center – $60,000
grant to support the Asian Child and Family Program, which provides
clinical, psychotherapy and interpreter services for Asian language-speaking
patients.
Joan Karnell Cancer Center
- Cancer Prevention and Early Detection
Outreach Program in the African American community - $41,000 grant for a second year of
funding for screening, genetic testing and education of men and
women in Philadelphia.
- Geriatric Oncology Program - $40,794 grant for education and outreach
efforts focused on symptom management, maintaining functional status
and independence and improving quality of life. This focus ensures
appropriate referral and enrollment for all eligible patients.
- Supportive Care Fund – $24,000
grant to assist families facing crises by providing financial
support for medication expenses, nutritional supplements, patient
and family living expenses, grocery vouchers, bus tokens, parking
and cab vouchers.
J. Edwin Wood Clinic – $25,000 grant for the renovation
and expansion of the clinic’s medical records and administrative
areas.
Pennsylvania Hospital Historic Preservation Grants
Save the Pine Building – $60,000 grant to preserve the nation’s
original and first hospital structure. This preservation campaign
is integral to not only maintaining the architectural value of
the Pine Building, but also integral to preserving the social associative
and commemorative value related to that period in history when
this important edifice was built. The Pine Building represents
the inception of structured scientific inquiry, medicine, medical
education and the provision of healthcare for the indigent and
mentally ill.
Historic Collections – $57,000
grant for the hospital’s
full-time archivist to arrange, describe and preserve the records
and artifacts of the hospital’s archives; to conserve rare
books; and to monitor the physical status/location of fine art
and furniture. |
